幽门螺杆菌耐药情况分析

目的调查贵阳医学院附属医院上消化道疾病患者中分离出的幽门螺杆菌(Helicobacter pylori,Hp)对常用抗生素的耐药情况,为临床选用抗生素治疗Hp提供实验室依据。方法收集2012年1月~2013年1月贵阳医学院附属医院胃镜室患者胃黏膜标本580例,微需氧环境培养5~7 d,对临床Hp菌株采用E-test法检测对5种常用抗生素(甲硝唑、阿莫西林、克拉霉素、左氧氟沙星、四环素)的耐药情况。结果 580例胃黏膜标本其阳性检出率为57.6%(334/580),污染例数为21株;经分离培养得到的313株Hp对甲硝唑、左氧氟沙星、四环素、阿莫西林及克拉霉素的耐药率分别为87.2%、46.9%、23.4%、25.5%和8.5%。结论贵阳医学院附属医院幽门螺杆菌对甲硝唑的耐药性较高,对克拉霉素的敏感性较好。

Objective To investigate the drug resistance of helicobacter pylori(Hp) separated from the patients in Affiliated Hospital of Guiyang Medical University with upper gastrointestinal diseases to the common antibiotic and to provide laboratory basis for the clinical treatment of Hp with antibiotic. Methods 580 gastric mucosa samples of patients in the gastro-endoscope room of Affiliated Hospital of Guiyang Medical University from January 2012 to January2013 were collected, then they were cultivated in the micro aerobic environment for 5-7 days, and the clinical drug tolerance conditions of the Hp strain to the 5 common antibiotics(Metronidazole, Amoxicillin, Clarithromycin, Levofloxacin and Tetracycline) were inspected with the E-test method. Results The positive rate of the 580 cases of gastric mucosa samples was 57.6%(334/580), the contaminated cases were 21 strains, and the drug tolerance conditions of the313 Hp strains after separation and cultivation to Metronidazole, Levofloxacin, Tetracycline, Amoxicillin, Clarithromycin were 87.2%, 46.9%, 23.4%, 25.5% and 8.5% respectively. Conclusion The drug tolerance of Hp to the Metronidazole is high and its sensibility to Clarithromycin is good.
