目的探寻针刺足三里治疗脑出血的作用机制。方法 96只SD大鼠随机分为假手术组(24只)、假针刺组(24只)、脑出血组(24只)、针刺组(24只),通过立体定位向脑内苍白球注入Ⅶ型胶原酶0.5U建立脑出血模型。针刺组针刺双侧足三里,假针刺组针刺大鼠双侧非经非穴处皮肤。观察脑缺血再灌注3、7、14d血管内皮生长因子(VEGF)表达的空间位置变化;检测各组脑缺血再灌注3、7、14d各时间点VEGFmRNA的动态变化。结果假手术组不同时间点VEGF未见明显表达。脑出血组、假针刺组和针刺组3d时在血肿周围均可观察到大量VEGF阳性微血管,7dVEGF阳性微血管开始深入血肿区,随后出现在血肿区。逆转录聚合酶链反应(RT-PCR)结果显示,VEGFmRNA在假手术组未见明显表达,脑出血后3dVEGFmRNA表达即明显增强,持续到14d,针刺组VEGFmRNA表达水平在各时间点均明显高于脑出血组,而假针刺组与脑出血组则水平接近。结论针刺足三里能促进脑出血大鼠脑内VEGF的表达,其可能参与脑出血损伤区的微血管系统重建,从而实现损伤组织的修复。
Objective:To investigate the mechanismsof Zusanli(ST36) in intracerebral hemorrhage.Methods:96 rats were randomly divided into the sham operation group,the sham acupuncture group,the intracerebral hemorrhage group and the acupuncture group with 24 in each group. The intracerebral hemorrhage model was established by injected 0. 5U collagenase type Ⅶ stereotaxically into globus pallidus in rats. The acupuncture group was needled Zusanli(ST36) . The sham acupuncture group was stimulated non-acupoints skins. The positional variations of the expression of vascular endothelial growth factor(VEGF) and the VEGF mRNA were observed at the 3th,7th and 14th day after model establishment. Results:There were no obvious VEGF expressions in different time points of the sham operation group. In the 3th day,a plenty of the microvessel with positive VEGF around the hematoma area were observed in the intracerebral hemorrhage group,the sham operation group and the acupuncture group. In the 7th day,these microvessels with positive VEGF were digging into the hematoma area. And then,they were appeared in this area. After detected by the RT-PCR,there were no obvious expressions of VEGF mRNA in the sham operation group. Its expression was more remarkable in the 3 days after intracerebral hemorrhage. This condition was lasted to the 14th day. In every time point,the VEGF mRNA expressions of the acupuncture group were significantly higher than those of the intracerebral hemorrhage group. Nevertheless,this expression in the sham acupuncture group was approximate to the intracerebral hemorrhage group. Conclusion:Needing Zusanli(ST36) do not promote the VEGF expression in the intracerebral hemorrhage rats,but also repair the injury of the microvessel of the hemorrhage damage area. This could be realized by the reconstruction of the vessel system.
