目的 探讨布加综合征患者肝脏的磁共振动态增强特征。方法 回顾性分析19例经DSA血管造影证实的布加综合征动态增强MRI表现。所有病例均采用FRFSE T2和FSPGR T1序列扫描,并行FSPGR T1多个时相动态增强扫描。结果 将19例病人门脉期图像的肝实质强化形式分4种类型:(1)中央强化型;(2)周边强化型;(3)斑片强化型;(4)均匀强化型。急性期4例,3例表现为中央强化型,1例为斑片状强化型;慢性期15例,4例表现为周边强化型,3例主要表现为斑片状强化型;8表现为均匀强化型。其他征象如肝肿大、肝脏尾叶增大、肝实质信号不均匀、肝内外侧枝血管形成;脾大腹水等表现也可见到。结论 多时相动态增强MR能评价布加综合征肝血流的动态变化。判断疾病的病程和估计肝实质受损程度。

英文摘要:

Objective To evaluate MRI dynamical characteristics of Budd - Chiari syndrome. Methods MR features of 19 cases of Budd -Chiari syndrome confirmed by DSA angiography were retrospectively analyzed. FRFSE sequence with T2 WI and fat saturation ,FSPGR with T1 WI and 3D dynamic enhanced MR were performed in all cases. Results The enhancement patterns were evaluated under g groups as following : ( 1 ) central; ( 2 ) peripheral; ( 3 ) patchy ; ( 4 ) homogeneous enhancement. Acute stage ( 4cases ) , 3 of them were central enhancement ,the other was suggested pathy enhancement. Chronic stage ( 15 cases) ,g of them were assessed peripheral enhancement,3 of them were pathy enhancement and 8 of them were homogeneous enhancement. The other signs such as hepatomegaly, enlarged caudate lobe, heterogenous singal intensity of hepatic parenchymal ,intrahepatic collateral vessels, extrahepatic collateral vessels and splenomegaly and ascites were seen. Conclusion 3D contrast - enhanced MRI is valuable in evaluating the changes of blood flow dynamically of Budd - Chiari syndrome in different stage, and help us to speculate the damage of liver and prognosis.
