目的检测HPVL1和16/18DNA在喉癌组织中的表达,探讨HPV在喉癌发生中的作用。方法用PCR检测123例喉癌组织及123例喉粘膜上皮组织中HPVL1和16/18DNA。结果在123例喉癌标本中,HPV—L1阳性率为76.42%;HPV-16E6阳性率为54.47%;HPV-16E7阳性率为58.54%;HPV-18E6阳性率为42.28%;HPV-18E7阳性率为48.78%.123例喉粘膜PCR结果表明:低危型HPV的阳性率与喉癌相近;高危型HPV的阳性率虽远远低于喉癌,但随着组织病变程度加重阳性率逐渐升高;HPV—L1的阳性率高于HPV-16E6、E7,随着组织病变程度加重,HPV—L1的检出率逐渐增高。结论HPV-16感染是重庆地区喉癌的重要发病因素;HPV-16感染后E6/E7片段的保留并持续表达与喉组织的癌变进程密切相关;HPV-16感染会增强喉上皮病变的风险,而HPV—18有协同作用;L1片段适合用于检测HPV感染。

英文摘要:

To look for the further evidence for HPV L1 -16 E6 and -18 E6 as carcinogenic factors in laryngeal carcinoma, we investigated their expressions in laryngeal carcinoma. We examined representative numbers of specimens from laryngeal cancer and controls with highly sensitive PCR technique for the presence of HPV L1 and high-risk types HPV16 E6 and HPV18 E6. PCR detection revealed that 76.42% samples were HPV L1 positive, 54.47% were HPV16 E6 positive, 58.54% were HPV-16E7 positive, 42.28% were HPV-18E6 positive, and 48.78% were HPV-18E7 positive. PCR screen of HPV infection in control crowd showed that the prevalence of HPV-16E7 was similar to that of HPV-16E6. The prevalence of HPV-16E6 was lower than that of laryngeal carcinoma. But, as the cells undergoing pathological progress, the HPV-16E6 positive rate was increased. The positive rate of HPV-L1 was the higher as compared with HPV-16E6/E7. Furthermore, the prevalence of HP V-L1 was significantly increased as the cells undergoing pathological progress. These results suggested that HPV-16 plays a significant role in the pathogenesis of laryngeal carcinoma; integration of E6 and E7 into host genome and stable expression of these genes may be associated with the carcinogenesis of laryngeal epithelium cells. In addition, HPV-16 and HPV-18 may synergistically function on the pathogenesis of laryngeal epithelium. And L1 segment is fit for examining HPV infection.
