目的应用功能磁共振成像和计算机图形辅助处理技术,探讨脑震荡后综合征患者大脑灰质体积的变化及其意义。方法选取2015年4月至2016年1月安徽医科大学附属省立医院神经外科收治的23例合并脑震荡后综合征的轻型创伤性颅脑损伤患者(试验组),同时招募资料相匹配的25名健康对照者作为对照组。两组均行相同参数的磁共振常规序列和高分辨率三维T1像扫描,利用统计参数图软件,对数据进行相关处理后,采用基于体素的形态测量学方法构建大脑结构网络,分割大脑灰质与白质结构,提取试验组(伤后3个月)与对照组大脑灰质体积的相关数据并进行统计学分析,从而发现脑震荡后综合征患者存在显著差异的脑区。结果与对照组相比,试验组左侧额下回、左侧颞下回、右侧额直回、左侧岛盖以及左侧壳核的灰质体积减小(均P〈0.001),未发现灰质体积增大的脑区。结论脑震荡后综合征患者的大脑灰质体积存在结构性异常改变,这种变化可能与脑震荡后综合征的临床表现有关。
Objective To study the volume changes of gray matter in patients of post-concussion syndrome and discuss its clinical significance using functional magnetic resonance imaging and computer-aided graphics processing technology. Methods Twenty-three patients diagnosed of post-concussion syndrome following mild traumatic brain injury admitted to Department of Neurosurgery, Anhui Provincial Hospital, Anhui Medical University from April 2015 to January 2016 were recruited as the test group. Twenty-five healthy subjects were recruited as the control group. All subjects underwent regular magnetic resonance scanning and high-resolution 3D-TI weighted examination with the same parameters. We built a brain structure network model, segmented grey and white matters, and analyzed the changes of gray matter volume with the method of voxel-based morphometry, intending to investigate the damaged cortical areas. Results Decreased gray matter volumes were found in areas including left inferior frontal gyrus, left inferior temporal gyrus, right gyrus rectus, left opereulum insu~ae and ~eft putamen( all P 〈0. 001 ) , while increased gray matter volumes were not observed. Conclusion Changes of gray matter volume were demonstrated in patients with post-concussion syndrome, which may be related to the pathophysiological mechanism of its clinical symptoms.
