目的 利用静息态功能磁共振成像(rs-fMRI)探讨脑震荡后综合征(PCS)伴记忆障碍患者后扣带回与相邻楔前叶以及全脑功能连接的情况及其意义.方法 选取2014年9月至2015年4月安徽医科大学附属省立医院神经外科收治的PCS伴记忆障碍患者28例(试验组),同期招募28名健康对照者(对照组).采集并处理rs-fMRI数据,分别以左、右后扣带回作为感兴趣区(ROI)与全脑进行功能连接计算并进行统计分析,得出PCS患者后扣带回与楔前叶及全脑功能连接的改变情况.结果 以双侧后扣带回为ROI,两组数据分析均提示后扣带回与内侧前额叶、扣带回中后部、楔前叶、顶上小叶等脑区有明显的强功能连接(均P <0.05).两组对比结果表明,以左侧后扣带回为ROI,左后扣带回与双侧楔前叶、扣带回中部及右后扣带回的功能连接呈增强状态(均P<0.05),而与双侧缘上回、颞上回、罗兰氏岛盖部、辅助运动区的功能连接呈减弱状态(均P<0.05);以右侧后扣带回为ROI分析,发现功能连接增强的脑区为双侧楔前叶,左侧额上回、额中回(均P<0.05),功能连接减弱的脑区为双侧缘上回、颞上回、罗兰氏岛盖部、辅助运动区、中央后回(均P <0.05).结论 静息状态下PCS患者后扣带回与额叶、颞叶、顶叶相关皮质的功能连接减弱可能是导致记忆障碍的原因之一,而与楔前叶功能连接显著增强可能是记忆障碍的代偿效应.
Objective To investigate the posterior cingulate,adjacent precuneus,and whole-brain functional connectivity and its significance in patients with post-concussion syndrome (PCS) combined with memory disorders using resting-state functional magnetic resonance imaging (rs-fMRI).Methods From September 2014 to April 2015,28 PCS patients with memory disorders (test group) admitted to the Department of Neurosurgery,the Affiliated Anhui Provincial Hospital,Anhui Medical University were selected,and at the same time,28 healthy controls (control group) were recruited.The rs-fMRI data were collected and processed.The right and left posterior cingulates were used as the region of interest (ROI) respectively to make whole-brain functional connectivity calculation and conduct statistical analysis.The changes of the posterior cingulate,precuneus and whole-brain function were derived in patients with PCS.Results The bilateral posterior cingulates were ROIs.The data analysis of both groups indicated that posterior cingulate had obvious strong functional connectivity with brain regions,such as medial prefrontal cortex,postmedian cingulate,precuneus,and superior parietal lobule (all P 〈 0.05).The comparative results of both groups show that the left posterior cingulate was used as a ROI,the functional connectivity showed an enhanced state in the left posterior cingulate and bilateral precunues,middle cingulate and right posterior cingulate (all P 〈 0.05);and the functional connectivity showed an decreased state in the bilateral supramarginal gyrus,superior temporal gyrus,Rolandic operculum,and supplementary motor area (all P 〈 0.05).The right posterior cingulate was used as a ROI,it was found that the functional connectivity enhanced brain areas were bilateral precunei,left superior temporal gyrus,and middle frontal gyrus (all P 〈0.05;and the brain areas with reduced functional connectivity were bilateral supramarginal gyri,superior temporal gyms,and Rolandic operculum,supplementary motor a
