目的应用静息态功能磁共振成像(RS—fMRI)探讨脑震荡后综合征(PCS)伴记忆障碍患者海马旁回的功能连接变化及其意义。方法收集2014年9月至2015年4月安徽医科大学附属省立医院神经外科收治的28例PCS伴记忆障碍患者(PCS组),同期招募28名健康对照者(HC组),采集并处理RS—fMRI数据,分别以左、右海马旁回作为感兴趣区(ROI),与全脑进行功能连接计算并做统计学分析,得出PCS患者海马旁回与全脑功能连接有改变的脑区。结果与HC组相比,PCS组左侧海马旁回与双侧前额叶、双侧颞叶、右楔前叶、右中央前回的功能连接减弱(均P〈0.05),与双侧枕叶、双侧舌叶、左顶上小叶的功能连接增强(均P〈0.05);右侧海马旁回与右额下回、双侧颞叶、双侧枕叶的功能连接减弱(均P〈0.05),与双侧额中回、左额下回、右额中回眶部、左颞下回、左枕叶中部、双侧角回、右舌叶、左顶上小叶的功能连接增强(均尸〈0.05)。结论静息状态下PCS患者海马旁回的功能连接存在异常,可能是导致PCS患者发生记忆障碍的原因之一。
Objective To investigate the alteration of functional connectivity and its significanee in postconcussion syndrome (PCS) patients with memory disorders using resting-state functional magnetic resonance imaging (rs-fMRI). Methods From September 2014 to April 2015, 28 PCS patients with memory disorders admitted to the Department of Neurosurgery, Anhui Provincial Hospital, Anhui Medical University and 28 health controls (HC) recruited in the same period were studied. The RS-fMRI data were collected and processed. The left and fight parahippocampal gyri were used as the regions of interest (ROI) to make functional connectivity with the whole brain and were analyzed in order to obtain the altered brain areas of hippoeampal gyrus and whole brain functional connectivity in patients with PCS. Results The functional connectivity of the left parahippocampal gyrus and bilateral prefrontal lobe, bilateral temporal lobe, right precuneus, and right precentral gyms reduced (all P 〈 0.05 ), and the functional connectivity with the bilateral occipital lobe, bilateral lingual lobe, and left superior parietal lobule enhanced ( all P 〈 0. 05 ). The functional connectivity of the right parahippocampal gyrus, right inferior frontal gyrus, bilateral temporal lobe, and bilateral occipital lobe reduced (all P〈0.05), and the functional connectivity with the bilateral middle frontal gyrus, left inferior frontal gyms, right orbital frontal gyrus, left inferior temporal gyrus, left middle occipital lobe, bilateral angular gyms, right lingual lobe, and left parietal lobule enhanced ( all P 〈 0. 05). Condusion In resting state, the abnormality of function connectivity of the parahippocampal gyrus in patients with PCS may be one of the reasons for resulting in memory impairment in patients with PCS.
