目的研究唑来膦酸对生物型全髋置换术后髋臼假体周围骨密度的影响。方法将50例符合纳入标准的行全髋置换的骨质疏松患者随机分为唑来膦酸组与对照组,术后10d唑来膦酸组给予注射唑来膦酸5mg,对照组予注射生理盐水100mL,术后均口服钙尔奇D 600mg/d及骨化三醇0.25μg/d,持续3个月。两组患者分别于术后1周、3个月、6个月及1年应用双能X线骨密度仪测定髋臼假体周围各分区的骨密度。结果获得完整随访的病例共45例,在术后3个月、6个月及1年,髋臼假体Ⅰ区骨密度均较术后1周升高,Ⅱ区于术后3个月两组均有轻微上升,之后降低,Ⅲ区骨密度始终呈下降趋势;唑来膦酸组Ⅲ区骨密度于术后6个月及1年时均显著高于对照组(P〈0.05),其余两区骨密度差异无统计学意义(P〉0.05)。结论唑来膦酸能有效防止非骨水泥型髋臼假体周围骨量减少。
Objective To investigate the early effect of zoledronic acid on BMD around acetabular prosthetic after cementless THA.Methods Fifty patients with cementless THA who met the inclusion criteria were randomly divided into zoledronic acid group(n=25)and control group(n=25).The zoledronic acid group was treated with zoledronic acid 5mg ten days after operation,and control group with saline solution.All patients were given oral Caltrate 600mg/d and Rocaltrol 0.25μg/d for three months.The BMD around acetabular prosthetic was measured by DEXA one week,3months,6months and 12 months after operation,and was compared between two groups by using t-test.Results A total of 45 patients completed the follow-up.BMD of ZoneⅠincreased in both groups postoperatively.BMD of ZoneⅡ had a slight increase in both groups in postoperative three months.BMD of ZoneⅢ declined continuously.BMD of the ZoneⅢin zoledronic acid group was significantly higher than that in control group in postoperative six months and one year(P〈0.05).Conclusion Zoledronic acid can effectively prevent the loss of acetabular bone density after cementless THA.
