目的研究立足于乡村一体化管理下,将乡村医生纳入乡镇卫生院统一管理,随着较低学历和低职称人员不断退休,补充正规医学教育和培训人员而实现对村医的替代,测算实现替代所需的年数,应补充医务人员数量和要求,以及应投入人力经费等支持系统,为上海市乡村医生服务模式的实践改革提供科学支撑。方法采用分层整群随机抽样的原则,对上海市8郊区(县)24个乡镇72个村720名乡村医生和312名农村卫生服务组织者和提供者展开现状和意向调查分析,以及137名村医日常工作日志的分析。结果和讨论如果采取一体化管理,乡村医生纳入乡镇卫生院统一管理,随着初中及其以下学历者的退休,不断补充正规医学教育和培训的人员实现逐步替代,则需完成替代年限是19年,每乡镇卫生院需承担年出诊次数3595次。同时,为提高乡村医生的收入和保障水平,则需增加人员经费7700.2万元,其中区财政3105.3万元,镇财政4010.9万元,村集体增加315.4万元,村卫生室收入增加268.6万元。
OBJECTIVE To explore how long to take, what quantity and quality for replacing doctors to require, and how much money to invest in doctors' income and social insurance by the government that integrating Management Town and Village Health Organizations, train rural doctors, substitute doctors educated normally with higher quality for rural doctors with their retiring, and make rural doctors normalized in Shanghai of China. METHOD By using stratified cluster and random sampling, the paper surveyed and analysis the willing of 312 health care organizers and providers and 720 village residents in 72 villages of 24 towns from 8 districts, and 137 rural doctors' work records in Shanghai. RESULTS and CONCLUSIONS Integrating Management Town and Village Health Organizations, train rural doctors, substitute doctors educated normally with higher quality for rural doctors with their retiring, and make rural doctors normalized needed 19 years and every town hospital need shoulder visiting a patient at home 3 595 times every year. 77 002 000 Yuan were increased to improve the rural doctors' income and social insurance.
