目的 调查河北医科大学第三医院骨科术后切口感染情况。方法 回顾性分析2 106例骨科术后切口感染患者相关资料。结果 骨科术后切口感染率为6.23%。切口感染病原菌:革兰阴性杆菌占65%,革兰阳性球菌32%,真菌3%。检出率较高的7种病原菌为金黄色葡萄球菌(27.87%)、铜绿假单胞菌(23.84%)、阴沟肠菌(12.25%)、大肠埃希菌(11.30%)、鲍曼不动杆菌(7.74%)、凝固酶阴性葡萄球菌(3.99%)、普通变性杆菌(2.85%)。引起感染的因素为开放性污染伤口、糖尿病、反复使用抗生素及住院时间过长。感染细菌中耐药细菌及比率:耐甲氧西林金黄色葡萄球菌(4.84%)、多重耐药鲍曼不动杆菌(1.19%)、超广谱β-内酰胺酶的细菌和头孢菌素酶(0.94%)。还有1例为泛耐药铜绿假单胞菌。结论 骨科手术后切口感染率较高,部分病原菌耐药性明显。
Objective To investigate the infection of incision in department of orthopedics of the third affiliated hospital of Hebei medical university. Methods The related data of 2 106 patients with incision infection after operation in department of orthopedics were retrospectively analyzed. Results The wound infection rate was 6.23% after orthopaedics operations. The main pathogens were Gram-negative bacteria (65%), the next was Gram-postive bacteria(32%), and the rate of fungus inecftion was 3%. The top 7 pathogens were staphylococcus aureus(27.87%). pseudomonas aeruginosa (23.87 %), enterobacter cloacae ( 12.25 %), escherichia coli ( 11.300%), baumanii(7.74%), coagulase negative staphylococcus (3.99%), and bacillus proteus vulgaris (2.85%). The causive factors for infecetrion were open infection wound, hyperglycemia, repeated usage of antibiotics, and long hospitalization. There were multi-resistant bacterias such as methicillin-resistant staphylococcus aureus (4.84%), multiresistant Acinetobacter baumannii (1.19%), produce ultra broad spectrum beta lactamase coli(0. 94%), and 1 case of pan-drug- resistant pseudomonas aeruginosa. Conclusion The infection rate of incision infection after operation in department of orthopedics was high, and the drug resistance of some pathogenic bacteria was obviously.
