目的研究局部应用50%葡萄糖降低异种全厚板层角膜移植术后炎性反应的作用。方法18周龄的健康雄性新西兰白兔16只随机分成两组,每组各8只。两组均进行猫兔异种全厚板层移植术:A组(实验组)手术后给予50%葡萄糖滴眼每132次,复方氯霉素滴眼液滴眼每133次;B组(对照组)术后仅给复方氯霉素滴眼液滴眼。每133次。术后每日在裂隙灯显微镜下观察植片存活情况,治疗及观察时间均为3个月。各组于术后1,2,3月各取2只兔的术眼角膜进行病理学检查。结果术后半个月内,A、B两组植片透明、无明显浑浊。术后20d部分植片开始出现排斥反应,在3个月内,A、B两组全厚角膜植片的排斥反应指数差异显著。在无排斥反应的正常兔角膜中组织学及电子显微镜下的结构特征清晰;但在出现排斥反应的兔角膜中有明显的角膜水肿和炎性细胞浸润。结论异种全厚板层角膜移植术后局部应用50%葡萄糖滴眼能减少术眼水肿,降低炎性反应,延长角膜植片成活时间。
Objective The effect of topical application of hypertonic glucose solution degraded the inflammatory reaction after xenogeneic full-thickness lamellar keratoplasty onto rabbits was investigated. Methods The cat xenogeneic full-thickness lamellar keratoplasty onto New Zealand white rabbit was set up, and was divided into two groups as follows. Group A (experimental group, n = 8) : 50% glucose solution was dripped on to the operated eyes of rabbits two times a day. Group B (control group, n = 8). All cat grafts were full thickness corneal graft. The compound chloramphenicol eye drops (chloramphenicol 0. 25 g, dexamethasone sodium phosphate 0.05 g and sodium chloride 0. 18 g per 100 ml) were given onto the operated eyes after operation three times a day. The operated eyes were observed daily with slit lamp microscope after surgery and histological and ultrastructure examinations were performed. The observation period was three months. Results The slitlamp microscope showed that the corneal grafts of group A and B were transparent within two weeks after transplantation. There were no significant differences in rejection index (RI) between two groups. The graft rejection appeared on the 20^th day. RI of fullthickness corneal implant in two groups was significant difference in three months. Histology and electron microscopy revealed the structural characteristics of normal clear cornea in the rabbits without rejection; But there was obvious corneal edema in the rabbits with rejection. Conclusions The topical application of 50% glucose can reduce corneal edema, degrade the inflammatory reaction and extend corneal graft survival time after xenogeneic full-thickness lamellar keratoplasty.
