目的研究强脉冲光(intensepulsedlight,IPL)联合外用5.氨基酮戊酸(aminolevulinic,AIA)对鲜红斑痣(portwinestains,PWS)动物模型鸡冠的损伤效应,探讨外用ALA提高IPL治疗作用的可行性。方法莱亨鸡30只随机分为3组,A组6只,B,c组各12只。A组:不做任何处理;B组:单纯IPL照射1次;C组:局部外用ALA后IPL照射1次。观察各组鸡冠形态学、组织学变化,测量毛细血管的损伤深度并计算毛细血管减少率;TUNEL染色法观察血管内皮细胞凋亡情况。结果B,c组鸡冠均退色明显。组织学观察可见毛细血管数目减少、管径变小,其毛细血管减少率分别为(67.92±1.79)%,(89.17±2.33)%,与A组比较,差异均有统计学意义(P均〈0.01);血管损伤深度分别为表皮下(235.25±3.27)μm和(373.77±4.32)μm。A,B,c组毛细血管凋亡指数分别为1.09±0.88,4.37±0.98和101.76±2.07,C组与A,B组比较,差异均有统计学意义(P〈0.01),C组毛细血管明显凋亡深度为皮下(759.19±0.33)μm。结论IPL与ALA的联合应用可能导致鸡冠真皮毛细血管的热损伤作用增强并诱导更深处血管内皮细胞发生凋亡,二者联用可以提高IPL的治疗作用。
Objective To investigate the damaging effect and possible mechanism caused by 5-aminolevulinic acid(5- ALA) combined with intense pulsed light (IPL) on vascular lesion of cock comb, which is the animal model of port wine stains (PWS). Methods A total of 30 leghorn chickens were divided into 3 groups randomly, with 6 in group A and 12 in others. Group A was the control group. Group B were treaded by IPL irradiation for one session, group C were treaded by IPL irradiation accompanied by external ALA cream. In the end,the morphology and tissue specimens of combs was observed, the depth of damage was measured and the decrease rate of the number of capillary vessels was calculated. Besides, TUNEL staining was conducted to observe the condition of capillary endothelium apoptosis. Results Blanching of the combs was achieved in groups B and C. Decreasing number of capillaries and waning vascular caliber were observed according to histological ob- servation, decreasing rate of the capillary vessels was ( 67.92% ± 1.79 ) % and ( 89.17% ± 2.33 ) % in group B and C respectively,which was lower than that in group A(P 〈0.01 ). Depth of damage in group B and C were ( 235.25 ± 3.27 ) μm and ( 373.77 ± 4.32 ) μm. The apoptosis index of capillary endothelium in group C was 101.76 ± 2.07, which was significantly lower than that in group A (1.09 ± 0.88 ) and group B (4.37 ±0.98) (P〈0.01).The depth of apoptosis in group C was (759.19 ±0.33)μm. Conclusion The combination therapy of topical ALA and IPL might enhance effects of thermal damage and induce apoptosis of capillary endothelium in dermis. Therapeutic effects are improved compared to IPL alone.
