Implementing family doctor system being the cardinal contents of nation new medicine reform and also being synthesize reformation target of medicine health system in Shanghai city.Based on author research,the concept of family doctor system and family doctor development were summarized for the first time.At the same time,the author discussed the intension and extension of family doctor system and put forward ＇54321＇ project of researching family doctor system.For example：5 certain-certain family doctor natural intelligence and certain providing service contents and certain need degree and certain service item expenses.4 squares pay-government to pay and basic medical treatment insurance to pay and the society to pay,and the individual to pay.3 square evaluation-providing evaluation and need evaluation and the third square evaluation.Finally,development strategy of family doctor system was established,and the development strategy means enhancing lead and innovation system and persist service and health development.
