目的:探讨2001-2010年某职工医院六类主要系统疾病患者十年间住院费用变化趋势,为减轻职业人群医疗费用提供依据。方法采用回顾性研究方法,收集2001-2010年某职工医院56655例住院患者病历资料,对各类疾病患者十年间住院费用进行趋势分析。结果恶性肿瘤患者人均住院费用和日均住院费用最高,分别为4452.38元和334.79元。呼吸系统疾病人均住院费用和日均住院费用最低。职业病的平均住院日最长,为25.15 d。经Spearman秩相关分析,2001-2010年六类系统疾病住院患者人均住院费用、日均住院费用与年份之间无相关性( P>0.05)。2001-2010年住院患者前十位单病种累计住院费用矽肺患者最高,为16933728.90元,其次依次为胆石病10184396.08元、矽肺并结核9894939.11元。预计到2013年住院患者矽肺的累积住院费用将达到2031338.23元。结论矽肺和恶性肿瘤患者的直接经济负担最重,应积极开展这两类疾病的预防保健工作,从根本上降低个人和社会的经济负担。
Objective To analyze changes in medical costs for inpatients in a Workers′Hospital in the past , aiming at providing a basis for more reasonable cost determination for inpatient services in Workers ′Hospitals in the future.Methods Med-ical records and data on hospital costs of 56 655 inpatients in six major departments in a Worker′s Hospital in Gansu Province be-tween 2001 and 2010 were reviewed and analyzed.Results During the 10 years analyzed , cancer patients had the highest aver-age total hospitalization expenses and average daily hospitalization expenses , which were 4 452.38 yuan and 334.79 yuan respec-tively.These two figures were lowest for patients with respiratory diseases.Patients with occupational diseases had the longest aver-age of hospital stay , which was 25.15 d.The Spearman′s rank-order correlation analysis showed that the average total hospitali-zation expenses and average daily hospitalization expenses for the six systemic diseases analyzed were not correlated with the calen -dar year between 2001 and 2010 ( P〉0.05 ) .Among the ten single disease entities analyzed , silicosis had the highest cumula-tive hospitalization costs (16 043 697.84 yuan), followed by cholelithiasis (10 125 971.75 yuan), and silicosis associated with tuberculosis (9 451 706.99 yuan) .It was estimated that cumulative hospitalization costs of silicosis inpatients would reach 2 031 338.23 yuan by 2013.Conclusion The direct economic burden was substantially high for inpatients , particularly for those with silicosis or malignant tumors , in Workers′Hospitals in Gansu Province in the past.Cooperative efforts are needed from all in-volving parts to reduce the social economic burden of inpatients in Workers′Hospitals in the future.
