磁共振弥散加权成像对超急性期脑梗死诊断价值的Meta分析

目的 通过Meta分析方法,比较磁共振成像(MRI)弥散加权成像(DWI)序列与T2WI序列对超急性期脑梗死的诊断价值。方法 检索CNKI与万方数据库1999~2013年发表的中文文献,按照诊断试验的纳入标准筛选关于MRI DWI和T2WI对急性期脑梗死诊断价值的诊断试验,评价纳入研究的质量,并采用Meta-Disc1.4软件进行Meta分析,绘制汇总受试者工作特征曲线(SROC曲线),计算曲线下面积(AUC)。结果 在诊断急性期脑梗死方面,MRI DWI与T2WI的灵敏度分别为0.94[95%CI 0.79~0.97]、0.33[95%CI 0.23~0.44];特异度分别为1.00[95%CI 0.94~1.00]、0.73[95%CI 0.5~0.85];AUC分别为97.48%、74.88%。结论 在诊断急性期脑梗死方面,MRI DWI序列具有较高的灵敏度、诊断价值更高。

英文摘要:

Objective To compare the value of MRI DWI and T2WI for diagnosis of superacute cerebral infarction through Meta-analysis. Methods Studies on diagnosis value of MRI DWI and T2WI for acute cerebral infarction in Chinese from 1999 to 2013 were selected from CNKI and WanFang database. Each article was critically appraised and screened, according to the criteria for diagnostic research published by Cochrane Method Group on Screening and Diagnostic Tests. Statistical analysis was performed by SPSS 18.0 and Meta-DiScl. 4. Heterogeneity of the included articles was tested, pooled weighted sensitivity and specificity and the corresponding 95% C1 were calculated. Summary receiver operating characteristic curve(SROC) was performed and the area under the curve (AUC) was calculated. Results The pooled weighted sensitivity and specificity and area under SROC curve for MRI DWI detecting superacute cerebral infarction was 0.94 (95 % CI O. 7900.97 ), 1.00 ( 95 % CI O. 94-1.00 ), 97.48%, respectively; for T2WI was 0.33 ( 95 % CI 0.23-0.44 ) ,0.73 ( 95 % CI 0.5 -0.85 ) ,74.88 %, respectively. Conclusion The study of meta-analysis for MRI DWI and T2WI in the diagnosis of superacute cerebral infarction showed that MRI DWI with higher sensitivity is of higher value in detecting superacute cerebral infarction.
