冷热刀系统既可对肿瘤组织实施快速降温冻结又可对其进行高温热疗,是新一代综合医疗设备。其治疗过程是在医学影像引导下将探针经皮插入至深部靶点部位,通过探针内冷媒和热媒的交替输送来确保探针工作段的冷冻及加热治疗,因此减少探针非工作段的漏冷或漏热、避免对沿途正常组织造成损害至关重要。本文针对冷热刀系统治疗模式的特殊要求,在探针非工作段引入一层具有良好生物兼容性及绝热性的薄壁管,以有效减少非正常冷量或热量的泄漏,并开展了相应的实验验证。同时,基于该方式建立了传热数学模型并推导出生物组织在探针作用下冷冻阶段的稳态温度场解析解,开展了相应的参数化研究,为今后进一步合理优化探针及套管提供依据。理论及试验研究均表明该方法能够简单有效地解决治疗探针的冷热保护问题,可望为今后实施安全高效的肿瘤冷热治疗创造条件。

英文摘要:

Cryosurgical/hyperthermia probe system (CHPS) is a new generation medical equipment capable of treating tumors through a combination between localized fast freezing and heating. This system performs its role by inserting the probe into the deep target tissues under the guidance of certain routine medical imaging systems. During the cycles of freezing and heating, it is very important to protect the normal tissues and important organs along the inserting track of the probe from injury. Taking account of the special requirements of CHPS, a method is proposed in this paper to avoid such adverse situation through attaching an adiabatic thin layer material along the wall of the non-working region of the probe. Preliminary experiments were performed to evaluate the present method. Meanwhile, mathematical model is developed to characterize the bioheat transfer of living tissues after frozen by the probe and corresponding analytical solution for the steady state temperature profile was obtained. Parametric studies were provided as a theoretical basis for the future optimization design of probe. Both theoretical analysis and experimental results demonstrate the feasibility of using adiabatic biocompatible thin layer as a simple, flexible and effective way to prevent cold or heat release from the probe. This provides a valuable strategy to perform a safe and efficient freezing and heating treatment on tumor in the near future.
