目的探讨焦炉逸散物接触对煤焦作业工人免疫调节因子的影响。方法采用职业卫生现场调查与实验室研究相结合的方法,测定作业环境中苯并(a)芘、苯溶物、焦炉逸散物等有害物质的质量浓度;选择91名焦炉作业工人作为接触组,80名无任何职业有害物质接触史的体检者作为对照组,采集空腹静脉血,用试剂盒检测血清中重要细胞免疫调节因子白细胞介素(IL)-2、IL-10、肿瘤坏死因子-α(TNF-α)的表达水平。结果焦炉作业工人工作场所存在职业有害因素,且炉顶有害物质质量浓度高于炉侧,存在职业有害因素;与对照组相比,焦炉作业工人血清IL-10、TNF-α水平均升高,IL-2水平则降低,差异有统计学意义(P〈0.05)。在接触组中,炉顶工组、炉底工组IL-10、TNF-α高于炉侧工组,差异有统计学意义(P〈0.05),IL-2水平差异无统计学意义(P〉0.05)。焦炉作业工人血清IL-10与TNF-α水平及工作区域有害物质质量浓度之间呈正相关,IL-2水平及工作区域有害物质质量浓度之间呈负相关。结论本研究提示焦炉逸散物的接触可以引起体内免疫系统分泌的相应免疫调节因子IL-10、TNF-α水平升高,IL-2水平降低。IL-10、TNF-α水平升高与IL-2水平降低可以在一定程度上作为机体的焦炉逸散物接触程度的指标。
Objective To evaluate the changes and significance of serum immune cytokine in workers exposed to coke oven emissions (COE).Methods The serum immune cytokine were measured respectively in 91 cases of coke oven workers (exposure group) and in 80 healthy adults without any occupational hazard exposures (control group) with ELISA method.Fasting venous blood sample was collected to measure serum IL-2,IL-10,TNF-α levels by test kits.Results There were statistically differences of IL-2,IL-10,TNF-α levels between two groups (P0.05).The serum levels of IL-10 and TNF-α in the exposure group were higher than those in the control group,but IL-2 was lower than that of the control (P0.05).The levels of serum IL-10 and TNF-α were positively correlated with the concentrations of hazardous substances in the working area,while IL-2 was oppositely correlated.Conclusion Exposure to COE can change the levels of cytokine secretion by immune cells,which could reflect the degree of exposure to some extent.
