目的研究寻常型天疱疮(PV)特异性抗原桥粒芯糖蛋白3(Dsg3)刺激下PV患者一级亲属T淋巴细胞的增殖情况,探讨PV的发生机制。方法Dsg3刺激PV患者一级亲属外周血单个核细胞(PBMC)体外培养3 d,BrdU掺入流式细胞术胞内染色,检测Dsg3特异性CD3+、CD3+CD4+、CD3+CD8+T细胞的增殖情况。结果PV抗体阳性亲属中PBMC经Dsg3刺激培养者与未经Dsg3刺激培养者比较,CD3+CD4+T细胞增殖率显著升高[(2.79±1.05)%vs(1.85±0.44)%,P〈0.05],PV抗体阳性亲属CD3+CD4+T细胞增殖率明显高于正常对照者[(2.79±1.05)%vs(1.25±0.71)%,P〈0.05]。结论PV患者一级亲属中PV抗体携带者体内存在Dsg3自身反应性CD3+CD4+T淋巴细胞,Dsg3释放可能在PV启动中起重要作用。
Objective To investigate the characteristics of T lymphocyte proliferation in first-degree relatives of the pemphigus vulgaris(PV)patients under the stimulation of Dsg3 and clarify the mechanism of the Dsg3-specific autoimmunity of PV.Methods We examined Dsg3 reactive peripheral blood T-cell subsets CD3+,CD3+CD4+,CD3+CD8+ T cells by BrdU incorporation assay and flow cytometry.Results After stimulated by Dsg3 antibody,the proliferative ratio of CD3+CD4+T lymphocytes in first-degree relatives with PV antibody positive was significantly increased [ (2.79±1.05)% vs (1.85±0.44)%, P 〈 0.05 ]. Dsg3-speciilc CD3+CD4+ T lymphocyte proliferation on PV-antibody- positive first degree relatives was significantly higher than that of healthy controls [ (2.79±1.05)% vs ( 1.25±0.71 )%, P 〈 0.05 ]. Conclusion The first-degree relatives of PV patients who carry the PV antibodies have Dsg3 autoreative T lymphocytes. The release of Dsg3 may play important roles in the initiation of PV.
