目的:探讨芎虫散偏汤治疗痰浊上扰型偏头痛的临床疗效。方法:将痰浊上扰型偏头痛患者88例,按随机数字表法分为两组,各44例。对照组予以西医治疗,研究组在西医治疗基础上予以芎虫散偏汤治疗,测定血清检测指标,同时对比两组患者临床疗效及并发症。结果:两组患者治疗前后比较,内皮素-1(endothelin-1,ET-1)水平降低,一氧化氢(NO)水平升高,白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)水平降低,5-羟色胺(5-hydroxytryptamine,5-HT)水平升高,偏头痛疼痛积分、发作次数及持续时间降低,脑血管血流速度改善,差异有统计学意义(P〈0.05)。与对照组比较,研究组治疗后ET-1水平较低,NO水平较高,IL-6、TNF-α水平较低,5-HT水平较高,偏头痛疼痛积分、发作次数及持续时间较低,脑血管血流速度改善更优,差异有统计学意义(P〈0.05)。对照组有效率72.73%,研究组有效率90.91%,两组比较,差异有统计学意义(P〈0.05)。结论:芎虫散偏汤治疗痰浊上扰型偏头痛疗效确切,能降低ET-1水平,提高NO水平,安全性高。
Objective:To investigate the clinical effect of Xiongchongsanpian Decoction in the treatment of migraine with turbid phlegm disturbing mind syndrome. Methods :88 patients with turbid phlegm type migraine were randomly divided into 2 groups ,44 cases in each group. The control group was given western medicine treatment, the study group was treated with Xiongchongsanpian Decoction based on the control group. The determination of serological monitoring indicators, and compared clinical efficacy and complications after treatment. Results:Compared with the control group, the levels of ET-1 decreased in two groups after treatment, levels of NO increased, levels of IL-6 and TNF-α decreased, levels of 5-HT increased, migraine pain score, attack frequency and duration of reduced cerebral blood flow velocity improved, and the difference was statistically significant ( P 〈 0. 05 ). Compared with the control group, levels of ET-1 in the study group after treatment were lower, levels of NO were higher,levels of IL-6, TNF- a were lower,levels of 5-HT were higher, migraine pain score, attack frequency and duration of low cerebral blood flow velocity were higher, and the difference was statistically significant (P 〈 0.05 ). The effective rate of control group (72.73 % ) was lower than the study group(90.91% ), the difference was statistically significant( P 〈 0.05 ). Conclusion : Xiongchongsanpian Decoction in the treatment of turbid phlegm disturbing mind type migraine has exact curative effect, which can decrease the level of ET-1 ,in- crease the levels of NO with high safety.
