目的 评价化疗患者应用PICC和静脉输液港的护理质量,为护理管理者明确PICC和静脉输液港应用中护理薄弱环节、以改善工作方法为临床筛选优质静脉化疗途径提供依据.方法 将由同一组护士护理的化疗患者120例随机分为PICC组、静脉输液港组,每组60例.采用自行设计中心静脉通道护理质量评价问卷调查表,调查表分为4部分:护士服务风貌、护士中心静脉通道基础护理水平、护士应用中心静脉通道应急护理能力、患者中心静脉通道应用效应.应用模糊综合评判法根据调查内容确定评判对象的因素集合、确定等级因素、建立评判矩阵、进行复合运算后,把每个评估项目的等级分累加得出K值,作为最终比较PICC和静脉输液港总护理质量评价的参考依据.结果 PICC组等级分累加K值=4.18,静脉输液港组等级分累加K值=4.37,两组护理质量评价等级介于较好和很好之间;但静脉输液港组K值> PICC组,表明静脉输液港总的护理质量强于PICC组.结论 护理管理者通过应用模糊综合评判法有效检测PICC和静脉输液港的护理质量,结合层次分析法确定权重系数评价PICC和静脉输液港的护理质量有利于管理者临床护理中作出客观、公正的评价,有利于管理者抓住护士群体中的薄弱环节进行分析,有针对性地制定培训计划,从而确定培训目标和内容,对优化护理队伍结构起到积极的作用,为临床筛选优质静脉化疗途径提高可靠依据.
Objective To evaluate the quality of caring for the patients with chemotherapy by PICC and VPA and to find the best way for chemotherapy.Methods A total of 120 patients were randomly divided into PICC group and VPA group.And 60 cases were in each group.Self - designed evaluation questionnaires on the quality of caring for center venous were adopted including the style of nursing service,the basic level of caring center venous catheter,the emergency care capacity of caring center venous catheter and the outcomes of patients with chemotherapy through center venous catheter.Fuzzy comprehensive evaluation method was adopted for analysis and K - value was used for the index of evaluating the quality of caring for chemotherapy by PICC and VPA.Results K was 4.18 of PICC group,while 4.37 of VPA group.Both of the groups of caring rating scale ranged between better and best.But k value of VPA group was higher than PICC group,which proved the quality of care of VPA was better.Conclusions Through fuzzy comprehensive evaluation method,nursing managers effectively checked the quality of caring of PICC and VPA in chemotherapy.Combining AHP,co - efficient to evaluate the quality of caring of PICC and VPA was determined,which was useful to an objective,unbiased assessment on the quality of nursing for chemotherapy through center venous catheter for nursing managers.And event chemotherapy method for patients.Usually that was useful to offering a best and reliable basis.
