人附睾蛋白4对慢性肾脏病的诊断价值

目的评价人附睾蛋白4(HE4)对慢性肾脏病(CKD)的诊断价值。方法收集CKD患者116例(CKD组),按照肾小球滤过率分为CKD1~2期31例,CKD3期36例,CKD4期16例,CKD5期33例;选择同期健康体检人群132例为对照组。检测CKD组各期和对照组血清HE4、肌酐和尿素水平,并通过受试者工作特征(ROC)曲线比较3个指标的诊断价值。结果 CKD组患者血清HE4、肌酐、尿素水平显著高于对照组,差异均有统计学意义(P〈0.05),且随着病情的加重,3个指标均明显升高;血清HE4的ROC曲线下面积(AUC)为0.981,大于肌酐(AUC为0.918)和尿素(AUC为0.880),对CKD的诊断价值最大。结论血清HE4对CKD有很好的辅助诊断价值,而且可以用来评估CKD病情的严重程度。

英文摘要:

Objective To evaluate the diagnostic value of human epididymis protein 4(HE4)for chronic kidney disease(CKD).Methods A total of 116 CKD patients were collected and divided into the stage 1-2 group(n=31),stage 3 group(n=36),stage 4 group(n=16)and stage 5 group(n=33)by glomerular filtration rate(GFR);contemporaneous 132 individuals undergoing the physical examination were selected as the control group.The levels of HE4,creatinine and urea were detected in various stages of CKD group and control group.The diagnostic values of three indicators were compared via the receiver operating characteristic(ROC)analysis.Results The levels of serum HE4,creatinine and urea in the CKD group were significantly higher than those in the control group,the difference was statistically significant(P〈0.05).Moreover,with the deterioration of CKD,the levels of three indicators were increased significantly;the area under the curve(AUC)of HE4 was 0.981,which was greater than 0.918 of creatinine and 0.880 of urea,the HE4 had maximal diagnostic value for CKD.Conclusion Serum HE4 shows an excellent auxiliary diagnostic value for CKD and can be used to assess the severity of CKD.
