目的:采用血氧依赖水平功能磁共振成像(BOLD-f MRI)技术,探讨不同类型、性质的脑肿瘤对大脑皮层运动区BOLD-f MRI信号激活范围、强度的影响。方法 :回顾性研究临近或累及大脑皮层运动区的48例脑肿瘤患者(恶性27例,良性21例)与10例正常人作为对照,获得功能区BOLD激活图像,分析大脑皮层运动功能区BOLD-f MRI信号范围、强度与脑肿瘤间的关系。结果:恶性脑肿瘤组信号范围及强度(范围:M I:51.30±9.27,SM I:29.56±9.29;强度:M I:29.59±12.28,SM I:23.00±6.42),良性脑肿瘤组信号范围及强度(范围:M I:100.57±29.60,SM I:79.00±11.85;强度:M I:46.00±8.58,SM I:47.24±14.93),正常人组信号范围及强度(范围:M I:192.00±23.83,SM I:177.40±21.53;强度:M I:62.20±8.15,SMⅠ:52.60±12.83)。三组间信号范围及强度差异具有统计学有意义(P〈0.05)。脑功能区成像范围与强度呈正相关(M I:R2=0.56,P〈0.05;SM I:R2=0.41,P〈0.05)。结论 :BOLD-f MRI能无创、快速地对皮层功能区进行定位,明确良恶性脑肿瘤大脑皮层运动区BOLD-f MRI的信号特征,具有较高的临床价值。
Objective: To discuss the effece of the different types, nature and origin of brain tumors on the blood oxygen level functional magnetic resonance imaging(BOLD-f MRI) signal in the cerebral cortex motor area,including the strength and the scope. Methods: 48 patients with neighboring or cumulative movement cerebral cortex brain tumors(27 malignant and 21 benign) and 10 cases without brain tumors were selected for the retrospective studies. The function BOLD activation images of motor cortex were obtained, and the motion function of cerebral cortex BOLD f MRI signal with strength and scope and the relationship between different brain tumors were analyzed. Results: Signal range and intensity of malignant brain tumor group(range: M I: 51.30 ± 9.27, SM I: 29.56 ± 9.29, strength: M I: 29.59 ± 12.28, SM I: 23.00 ± 6.42), the signal range and intensity of benign brain tumor group(range: M I: 100.57 ± 29.60, SM I: 79.00 ± 11.85, strength: M I: 46.00 ± 8.58, SM I: 47.24 ± 14.93),the signal range and intensity of normal group(range: M I: 192.00 ± 23.83, SM I: 177.40 ± 21.53, strength: M I:62.20 ± 8.15, SM I: 52.60 ± 12.83). Results of the signal range and intensity in three groups is different(P〈0.05). Brain function imaging range and intensity were positively correlated(M I: R2= 0.56, P〈0.05, SM I: R2=0.41, P〈0.05). Conclusion: BOLD f MRI is a noninvasive test and fast to locate the cortex function, which can play a reference role in the diagnosis of benign and malignant brain tumor before operation.
