多囊卵巢综合征中医证候与血清激素水平的相关性研究

目的:研究多囊卵巢综合征(PCOS)患者血清激素水平与中医证候的相关性。方法:100例PCOS患者按中医证候分为肾虚(37例)、肝郁(34例)、痰阻(29例)3组,分别用化学发光法测量卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、催乳素(PRL),比较不同中医证候之间血清激素的变化;比较各证型患者中医证候积分与血清激素的相关性。结果:痰阻型患者LH与肾虚型和肝郁型相比均有差异(P〈0.05);而肝郁型患者的E2水平与肾虚型和痰阻型有差异(P〈0.01和P〈0.05)。肾虚型患者的证候积分与FSH有相关性(r=0.336),而肝郁型患者的证候积分与E2有密切的相关性(r=0.863)。结论:血清激素在PCOS的发生中起重要作用,从中医辩证分型来看LH与痰阻型有关,FSH与肾虚型关系密切,而E2与肝郁型关系最为密切。

英文摘要:

Objective: To research the correlation between symptom and sign of Traditional Chinese Medicine (TCM) and sexual hormones in polycystie ovary syndrome (PCOS) . Methods: 100 patients with PCOS were divided into 3 groups as kidney deficiency ( n = 37 ), congestion of liver ( n = 34) and phlegm stagnation ( n = 29) groups according to symptom and sign of TCM, estimated FSH, LH,E2, PRL by chemiluminescence to compare hormones and the correlation between score of TCM and hormones in each group. Results: Con- tents of LH were increased in phlegm stagnation group compared with that of kidney deficiency and congestion of liver groups ( P 〈 0. 05 ) , contents of E2 were increased remarkable in congestion of liver group compared with those of kidney deficiency and phlegm stagnation groups (P 〈 0. 01 and P 〈 0. 05 ). The score of TCM in kidney deficiency group related with FSH ( r = 0. 336). The score of TCM in congestion of liver group related with E2 (r = 0. 863) . Conclusion: Sexual hormones play a key role in the pathogenesis of PCOS. LH has a connection with phlegm stagnation syndrome of TCM and FSH has a connection with kidney deficiency syndrome of TCM, E2 has the closest connection with congestion of liver syndrome of TCM.
