目的考察滤除白细胞对全血保存质量和功能的影响。方法1袋(2U=400mL)新鲜全血均分为滤除白细胞全血(简称滤白)组:按照去白滤器的使用说明书操作,在采血〈6h过滤制备去白细胞全血;对照组:不滤除白细胞。过滤及未过滤的全血均采用无菌方式分别分为6小袋[(30—40)mL/袋],(4±2)℃常规保存,分别于保存1、7、14、21、28、35d各取1小袋,检测血常规、pH、游离血红蛋白(rHb)、氧亲和力(P50)、ATP、2,3-二磷酸甘油酸(2,3-DPG)、电解质、红细胞膜表面PS、CD47表达、葡萄糖、乳酸等指标,比较过滤与未过滤全血质量的差异。结果实验组1u全血过滤后剩余白细胞数为(0.32±0.05)×10^6个;滤白组和对照组比较,保存35d时血红蛋白(Hb)(g/L)为149.00±11.53vs137.20±3.96、MCV(fL)为98.90±3.23似98.96±2.10、pH为6.78±0.02vs6.75±0.02、Pso(mmHg)为15.36±0.63vs15.18±0.22、ATP(1a,mol/gHb)为3.57±0.15vs3.61±1.02、2,3-DPG(mmol/L)为0.13±0.09vs0.22±0.2、PS阳性率(%)为0.96±0.08vs1.19±0.03、CIM.7阳性率(%)为47.65±3.92vs48.37±4.35(P〉0.05),溶血率(%)为0.08±0.02vs0.12±0.03(P〈0.05);保存28d时乳酸(mmol/L)为15.76±0.19vs18.73±0.97、葡萄糖(mmool/L)为20.21±0.55vs18.57±0.46(P〈0.05)。结论全血离体保存期间会发生一系列质量和功能的变化,滤除白细胞全血的溶血率、乳酸、葡萄糖等指标略优于未过滤的全血,二者的携释氧功能及其他生理生化指标均无明显差异,说明滤除白细胞未对全血保存期间的质量产生不良影响,并可能有利于血液保存。
Objective To explore the effects of leukodepletion filtration on the quality of whole blood. Methods Each of 5 units of fresh whole blood (400 mL) were equally divided into 2 aliquots. One subunit was used as filtered group which was filtered by leukocyte deplete filter within 6 hours after blood collection, while the other unit of the pair was used as the control without filtration. Each subunit of 2 groups was separated into 6 parts ( 30 - 40 mL each) in sterile eondition and stored at ( 4 ± 2 ) ℃ for 35 days. Blood routine, pH, free hemoglobin ( FHb), Ps0, adenosine triphosphate ( ATP), 2,3-diphos- phate glycerie acid (2,3 -DPG), electrolytes, the expression of PS and CD47 on red cell membrane, glucose and lactic acid of blood were evaluated on the 1 st, 7th, ]4th, 21st, 28th and 35th day, respectively. Then the storage qualities between the 2 groups were compared by statistics. Results The number of residual white blood cells after filtration was ( 0. 32 ±0. 05 ) × 106 . The values of HGB( g/L), MCV ( fL), pH, Ps0 ( mmHg), ATP( μ mol/gHb ) ,2,3-DPG( mmol/L), the expression of PS ( % ) and CD47 ( % ) of filtered group and control group on the 35th day were as follows : ( 149.00 ± 11.53 ) vs ( 137.20±3.96) ,(98.90±3. 23) vs(98.96 ±2. 10) ,(6. 78±0. 02)vs(6. 75 ±0.02), (15.36 ± 0. 63) vs(15.18±0.22), (3.57± 0. 15)vs(3.61±1.02) ,(0.13±0.09)vs(0. 22 ±0.20), (0.96 ±0.08)vs(1.19±0.03), (47.65 ±3.92)vs (48.37 ± 4. 35 ) ( P 〉 0. 05 ). The hemolytic rate of filtered group and control group were ( 0. 08 ±0. 02 ) % vs ( 0. 12 ±0. 03 ) %. There was a significant difference. The amount of lactate(mmoL/L) of filtration group and control group on the day 28 were ( 15.76 ±0. 19) vs ( 18. 73 ±0. 97 ), while the glucose (mmol/L) of these groups were (20. 21 ±0. 55 ) vs ( 18.57 ±0.46 ). There were significant difference in lactate and glucose.
